  2. Your personal data will be processed to provide psychotherapeutic services.
  3. You provide all data yourself; I do not obtain data from other sources.
  4. The legal basis for the processing of your data is: Article 6(1)(c), (d),  Article 9(2)(c), (h), and Article 9(1)(h) of the RODO, in connection with the provisions of the Act of April 15, 2011 on therapeutic activity (Journal of Laws of 2018, item 160, as amended) and the Act of November 6, 2008 on Patient Rights and Patient Ombudsman (Journal of Laws of 2017, item 1318, as amended).
  5. Recipients of your personal data will be the only entities authorized to obtain personal data under the law.
  6. Your personal data will be stored under the legal regulations in force in this regard.
  7. You have the right to request from the controller access to your personal data, rectification, deletion, or restriction of processing, the right to object to processing, and the right to data portability. Such requests will be considered as long as the prerequisites outlined in the applicable laws and regulations exist. You have the right to complain to a supervisory authority.
  8. I do not use automated decision-making, including profiling. Your personal data is needed for record-keeping in the form of a client card, and for visit management purposes.
  9. I contact you at the phone number or email address you provide to confirm, change or cancel appointments (Art. 6(1)(b) and (f) RODO, as the so-called legitimate interest of the administrator, which is the perioperative care of the patient and more efficient management of schedules, and Art. 9(2)(h) RODO, as the so-called provision of health care and management of health care systems and services in connection with Art. 3(1) of the Law on Medical Activity Art. 24 of the Law on Patients’ Rights).
  10. As an entrepreneur, I also keep accounting books and have tax obligations – I issue bills for services rendered, for example, which may involve the processing of your personal data (Art. 6(1)(c) of the RODO in connection with Art. 74(2) of the Accounting Act of September 29, 1994).
  11. I process all data processed for accounting purposes and for tax reasons for 5 years calculated from the end of the calendar year in which the tax obligation arose.
